Provides case management to help seniors if they are overwhelmed with managing their health care needs. Also offers in-home assessments for seniors who may need extra help at home by providing in-home personal care services such as bathing, dressing, personal hygiene and mobility assistance or light housekeeping, shopping etc. This program may also provide assistance in paying for Lifeline service (personal emergency response system).

Case Management and In-home assessment, Expanded in-home Services for the Elderly Program (EISEP), Family Caregiver support & respite services, HIICAP's “Health Insurance Information, counseling & assistance program, Home Energy Assistance Program (HEAP) and residential repair, legal services, Project Life Saver International Bracelet Program, and Medicare Prescription Drug Program, C.A.R.E.S. Unit which is the point of entry in Franklin County for information and access to long term care services.
