Individuals ages 18-64 with a documented TBI and reside in Broome, Chemung , Schuyler, Steuben, Tioga, or Tompkins County.

Provides supports and services through the Department of Health Traumatic Brain Injury (DOH-TBI) Waiver Program. The service works in partnership with the individual to secure supports and services that will allow them to live as independently and productively as possible while managing symptoms of TBI or related condition.
