Eligibility

Must be a resident of Western New York, 65 years of age or older, and a U.S. Citizen who meets one of the following requirements:

Has an annual income less than $38,000 for a household of one or less than $44,000 for a household of two

Is a permanent resident of a care facility

Must be cognitively, emotionally, and physically capable of communicating and experiencing the wish, and must be unable to fulfill the wish on one's own. Service Area: WNY.
