Offers support to Nassau or Suffolk County residents recovering from substance abuse transitioning from inpatient treatment back home or into a sober living environment. Assists individuals in recognizing relapse triggers, addressing thoughts, beliefs and feelings and establishing a local support system. Develops an individualized relapse prevention plan.
