Provides an individualized plan of care for terminally ill patients and their families through a coordinated interdisciplinary team of skilled professionals and volunteers who offer social, emotional and spiritual support; home-care program with short term in-patient support pain management and symptom control with 24 hr. access to care; education, guidance, support and respite for caregivers bereavement care nutrition counseling physical, occupational, speech and respiratory therapy medical equipment and supplies; short-term hospital care.
