NY Connects: Seniors 60 and older and individuals with disabilities residing in New York State.

Provides EISEP Case Management, Personal Emergency Response Systems (PERs), In-Home Contact and Support, Health Insurance Information Counseling Assistance Program (HIICAP), Home Delivered Meals (HDM), Congregate Meals, Nutrition Counseling, Nutrition Education, Information and Assistance, NY Connects Options Counseling, Afternoon Trips and Transportation, and Senior Center Recreation and Education. Has a contract with the Center for Elder Law & Justice for Legal Assistance.
