Eligibility
Nassau and western Suffolk county residents, ages 6 to adult, with mental health, substance use and co-occurring disorders. Includes family members and significant others. Individuals under 18 years of age must be accompanied by a parent/legal guardian for intake and psychiatric assessments. An adult must be present on site during all sessions. *Appointment required for AOT (Assisted Out Patient Treatment), hospital and inpatient adolescent, children and Medicare referrals.

Offers outpatient treatment services to adults, adolescents, children and their significant others living in Nassau and western Suffolk counties, whose lives have been affected by mental health, substance use and co-occurring disorders. Provides a full range of treatment services including comprehensive and psychiatric assessments, psychiatric medication management, medication assisted treatment for substance use disorders, individual and group counseling. Offers Suboxone and Vivtrol inductions,ambulatory outpatient detoxification services, on-site primary care assessment and monitoring. Also provides targeted case management, psychiatric rehabilitation, crisis stabilization and 24 hour crisis services, peer support services as well as support groups for families and their significant others. Offers services for members of the military, their families and veterans.
