* Provides onsite health and wellness programming and service coordination to adults 55+ who have active Medicaid, a chronic health condition and are homeless.* Offers a rental subsidy for participants who reside at one of the two locations and meet the qualifications for the ESSHI.* Offers transportation to participants for planned shopping and to off-site Supportive Housing sponsored health and wellness programming.
