Provides short-term skilled care and support services at home for patients with disabilities and complex medical conditions from newborn to 44 years of age. Offers a family-centered, team approach to caregiving with the goal of improving patients' independence and quality of life. Treats patients with a wide spectrum of health conditions including Cerebral Palsy, HIV/AIDS, seizures, Failure to Thrive, Hydrocephalus, Diabetes and Asthma. Offers physical, occupational, speech and respiratory therapy, specialized pediatric nursing, private duty nursing, home health and personal care aides, skilled nursing visits, palliative care, medical equipment and supplies.
