Eligibility
Youth between 12-17 years of age at risk of severe system consequences due to serious externalizing, anti-social, and/or delinquent behaviors. Exceptions for youth ages 10-11 and 18 can be requested.

Provides a behavioral-based community program for families that have a child from ages 12 to 17 who is at risk for out-of-home placement. Works to decrease a youth's involvement with the juvenile justice system by addressing truancy, substance abuse, criminal activities, or other antisocial behaviors.

Provides residential and nonresidential services that include primary care, behavioral healthcare, crisis services, integrated care management, peer services, and early intervention. Offers services related to substance use disorder, traumatic brain injury, intellectual disabilities, child welfare, and juvenile justice. Also provides crisis respite, HIV/AIDS homeless housing, domestic violence shelter, and crime victim assistance.
