Description
Provides help and support in accessing, medical and dental care, specialty care, housing, social security disability income, supplemental social security income, Medicaid, AIDS drug assistance programs, emergency financial assistance, mental health treatment, legal assistance including health care proxy, living wills, and permanency planning. HVCS also works with clients who are without Medicaid or private insurance. We do this through grant funding. Each client in need of case management services is assigned a single case manager, receiving services through office and home visits.

Hudson Valley Community Services provides the people of New York’s Hudson Valley region HIV/AIDS services, SNAP access and education, van-based drug prevention services, syringe exchanges, food pantries, housing support and cognitive behavioral interventions to decrease recidivism among jail inmates at no direct costs to the clients.
