Clients must be a patient of one of NLS's local healthcare partners: Evergreen Health, Catholic Health/Catholic Charities, Roswell, or the US Department of Veteran Affairs (VA). Financial eligibility will be determined at intake. Service Area: Erie, Genesee, Niagara, Orleans, & Wyoming Counties.

A free legal service program integrating legal expertise into healthcare settings to address social determinants of health and structural inequities facing patients and their care providers, in order to assist in removing barriers to positive health outcomes. - Housing (Eviction, Section 8, Habitability) - Family (Custody, Support, Orders of Protection, Divorce) - Public Benefits (SNAP, DSS, Temporary Assistance) - Disability (SSI, SSDI, Return to Work) - Estate Planning (Wills, POA, Healthcare Proxy) - Name Changes - Veteran Issues (Discharge Upgrades, Service-Connected Disabilities) - Referrals ##REENTRY##
