Eligibility
Individuals ages 18 to 64 years and:
  • have active Medicaid
  • assessed to need nursing home level of care but choose to reside in the community
  • have a primary diagnosis of TBI or similar non-degenerative condition OR diagnosed with a physical disability OR 65 years of age and older
  • enrolled in NYS TBI or NHTD waiver programming

Provides day habilitation services for individuals with a traumatic brain injury and individuals 60 years and older. Services include activities to help preserve mobility and cognitive skills.
