Application process

Call 211 to schedule an appointment. Must bring original Social Security cards or individual taxpayer ID for you, your spouse, and dependents; photo ID for self and spouse; all W-2 and 1099 forms, information for other sources of income, and information for all deductions/credits; a copy of last year's return; total paid to day care provider and their tax ID number; birthdates for everyone in the household; and form 1095A, B or C (Affordable Health Care Statements). For married couples filing jointly, both spouses must be present. For married couples filing separately, only one spouse needs to be present, but must provide the other spouse's personal information.
