Application process
Call 211 to schedule an appointment. Must bring Social Security cards for all individuals who will be on the return and photo ID for all adults on the return; all W-2 and 1099 forms, information for other sources of income, and information for all deductions/credits; a copy of last year's return; total paid to any day care providers and their tax ID number; birth dates for everyone in the household; and form 1095A, B, or C (Affordable Health Care Statements). For married couples filing jointly, both spouses must be present. For married couples filing separately, only one spouse needs to be present, but must provide the other spouse's personal information.
