* Provides a 24-Hour skilled nursing care and rehabilitative services that considers each person's unique strengths and needs to develop comprehensive, individualized care plans, including: Physician services. Personal care. Occupational therapy. Physical therapy. Registered dietitians. Social work services. Pharmacy services. * Offers a transitional care center that is designated for patients requiring a short stay following an illness, accident or injury. Services include: Orthopedic rehabilitation. Stroke rehabilitation. Cardiac rehabilitation. IV therapy. Recovery from other medically related hospitalization. * Offers a memory care center that serves individuals in various stages of Alzheimer's disease and related conditions. The center is designed to: Focus on an individuals strengths. Enable individuals to maintain cognitive and functional abilities for as long as possible. Provide ample space for specialized programs. * Offers a hospice care for those individuals who have chosen a comfort care only approach to end of life care.
