Description

* Provides assistance in maintaining an independent lifestyle for individuals with disabilities with the following resources:

Self-Direction - for persons with developmental disabilities
Medicaid Service Coordination
Habilitation Services - assistance with daily tasks and independent living skills
Consumer Directed Personal Assistant Program
Assistance in transitioning from nursing care facilities to the home (including moving assistance).
Respite vouchers to allow caregivers to attend to their personal needs.
Financial reimbursement for items that allow an individual with disabilities to remain in their home.
