Our friendly and experienced Navigators assist individuals and families searching, selecting and applying for public health insurance programs (e.g. Medicaid, the Essential Plan, and Child Health Plus) and commercial health insurance products using the New York State of Health: NY's Official Health Plan Marketplace.
The mission of the Healthcare Consortium is to increase access to healthcare and support the health and well-being of the residents in Columbia and Greene Counties. We address the barriers--whether they be physical, financial, or practical--that people confront when trying to get and stay healthy. Services provided include: non-emergency medical transportation through our Children and Adults Rural Transportation Service (CARTS); health insurance enrollment services through our Navigator Program; information, referral and assistance for individuals in need of long-term services and supports through our NYConnects program; access to no or low-cost prescription medications; financial assistance for those who are experiencing hardship due to illness or injury; and what we call "way-finding," helping people to find their way through the healthcare maze.
