- Provides clients and other individuals identified through outside community agencies with health and independent living skills development, psychosocial support services, supportive housing coordination, non-intensive case management (supportive case management) and housing placement assistance and referral. These services are provided with several goals, including resolution of immediate crises and referral to long-term services as needed; increased attendance of medical appointments and adherence to health care plans; and placement into permanent housing.
