Description
Helps residents of Columbia, Greene and Northern Dutchess Counties obtain needed prescriptions if they are unable to afford them or lack prescription coverage. Provides access to low or no cost prescription medications by the provision of direct financial assistance, assistance applying to patient assistance programs, and information and referral to other resources.

THE HEALTHCARE CONSORTIUM
The mission of the Healthcare Consortium is to increase access to healthcare and support the health and well-being of the residents in Columbia and Greene Counties. We address the barriers--whether they be physical, financial, or practical--that people confront when trying to get and stay healthy. Services provided include: non-emergency medical transportation through our Children and Adults Rural Transportation Service (CARTS); health insurance enrollment services through our Navigator Program; information, referral and assistance for individuals in need of long-term services and supports through our NYConnects program; access to no or low-cost prescription medications; financial assistance for those who are experiencing hardship due to illness or injury; and what we call "way-finding," helping people to find their way through the healthcare maze.
