A comprehensive screening process by Information and Assistance Specialists provides personalized options counseling to help consumers make informed decisions and assist in accessing available Long Term Services and Support options. Information and assistance provided to older population, disabled individuals of all ages, and caregivers. Information and assistance is available in various ways including telephone access; face-to-face meetings at the office, in the community, or in the home; and via email. Also provides an online resource directory of providers of long term services and supports, information about the different types of such services, and contact information for the local programs; directory available at www.nyconnects.ny.gov. Serves as the point of contact for several other programs run through the Department of Senior Services, such as the Senior Farmers' Market Nutrition Program, which provides low-income seniors with coupons that can be exchanged for eligible foods (fruits, vegetables, honey, and fresh-cut herbs) at farmers' markets, roadside stands, and community supported agriculture (CSA) programs. Provides a $150 gift card to ALDI, Price Chopper, or Tops and places a 30-day hold on termination for National Grid customers who are behind on their electric bill. This assistance is available for each customer only once per program year.
Offers help in emergencies with such basic needs as food, prescriptions, housing, and heat. Offers a parenting program for divorced or separated parents. When funding is available, provides FEMA assistance for one month of rent or mortgage payments as long as the tenant or owner can demonstrate the ability to make future payments on their own; funding currently unavailable. Utility assistance is not available through these funds.
