MEALS FOR CONSUMERS LIVING IN SUPPORTED HOUSING | THE REHABILITATION INSTITUTE

Provides vocational opportunities to people with disabilities, seeks to maximize their independence and adjustment to the community, offers job placement in a competitive position. Individuals prepare and deliver hot meals, via a mobile unit from TRI, to needy individuals living in supported housing. Documentation of disability and parental consent for individuals under 18 years old required.

SUPPER HUGS | BREWSTER CARES - WARMING CENTER

Connect local restaurants to families in the community in need of a meal. Whether a family is having financial difficulties, illness, a death in the family, or anything else bringing them down, we want to be sure they know their community supports them. Supper Hugs have delivered 1,000 meals to families through this program so far.

COMMUNITY FAMILY SUPPORT | LARCHMONT FRIENDS OF THE FAMILY

Provides confidential assistance to families dealing with a catastrophic illness or a sudden death of an immediate family member. An assessment is completed to better serve the needs of each family. Services can be preparing meals, arranging transportation to medical appointments, working on children's educational needs, providing information on resources in the community, financial support, etc.

Provides congregate meals and home delivered meals for seniors 60+ years. Social, cultural, educational and recreational activities also offered. Funded through Suffolk County Office for the Aging and Southampton Town.

OUTREACH- PARISH SOCIAL MINISTRY | PARISH SOCIAL MINISTRY

Provides a variety of emergency services for food, clothing, financial assistance, shelter, and job information. Other services are Bereavement Support Groups Divorce/Separation Support Group Home Visits, chores, and phone visits to homebound Lend-A-Helping Hand for women with breast cancer Lending services of medical items (i.e. wheelchairs, walkers, etc.) Meals on Wheels Senior Citizen Services information Thanksgiving Ministry Thrift Store Transportation to and from medical appointments and personal needs (a van with a lift is also available). Bagels and Bingo is a monthly program to bring homebound seniors out to the Parish Center in cooperation with the Rosemary Kennedy BOCES School. Call Ministry's coordinator for further information regarding all services.

HOME DELIVERED MEALS | CENTRAL ISLIP SENIOR CENTER

Delivers frozen meals weekly to those who are unable to prepare meals, have difficulty shopping for food and have no one available to assist them while they are incapacitated or recuperating from an illness or accident. Eligible seniors can receive a total of 5 meals weekly from the Congregate Nutrition Program, the Home Delivered Meal Program or a combination of the two. A voluntary contribution of $2.00 per meal is suggested. Meals will not be denied if no donation can be made. Please call for information.

MANAGED LONG TERM CARE | EVERCARE

Evercare provides managed long-term care for those individuals who are unable to live independently, and who require medical management and supervision as well as assistance with daily living needs. These services include (but not limited to): skilled nursing, home health and personal care aides, non-emergency transportation to and from medical appointments. Nutritional evaluation and planning, home delivered meals, Durable Medical Equipment (DME)/supplies, day program - social model, social and environmental supports, physical, occupational and speech therapy, dental, vision, audiology and podiatry services, respiratory therapy.

Sign up to receive a bag of food packed and delivered by our volunteers. Just fill out the form or call. Deliveries will be on Monday through Friday either morning or early afternoon. Bags will be left outside and you will be called to let you know it has arrived.
