Woodlawn Commons offers 60 independent one-bedroom and two-bedroom apartments for seniors as well as 40 enriched living apartments for those who might need additional services. Special features include gardens, a hair salon, a country store, a wellness center, arts and crafts, a social activity area, scheduled transportation, educational-cultural events and a private family dining room. A library with a fireplace is also available. There is 24-hour security, and interconnecting corridors between apartments and the community center There is a laundry-refuse area in each building and ample parking. This is located on the public bus line and is within walking distance of downtown Saratoga Springs. Pets are allowed.

Eligible individuals can receive information and referrals for Expanded in-home services for the Elderly Program and Medicaid services such as:- Referrals for home care services (personal care aides and consumer directed personal assistants) for low to moderate income levels for Tompkins County residents, including senior citizens and residents of any age with disabilities who need long-term care.- In-home assessment for an evaluation to determine specific Long Term care needs along with case management for ongoing assistance and monitoring of these services- Assistance with finding appropriate residential care.- Guidance through the process of nursing home placement by completion of Patient Review Instrument (PRI), a nursing assessment required by New York State for nursing home placement.- Referrals to local adult care homes.Traumatic Brain Injury- Assistance to referral to Traumatic Brain Injury waiver service, including completion of ongoing assessments required for recipients. Nursing Home Transition Diversion- Assistance with referral to waiver service, and completion of ongoing assessments required for recipients.Private Duty Nursing- provide information for application process and services, for individuals who require continuous skilled nursing care.Referrals for Personal Emergency Response System, FoodNet, HEAP and WRAP.Family type home- provide information for requirements of starting a family type home.

Self Direction:A service option that allows you, the individual with developmental disabilities, to choose the mix of supports and services that work best for you, how and when they are provided, and the staff and/or agency that provide them. Support Brokerage and Fiscal Intermediary are required components of Self Direction.Support Broker:A professional who educates the person and family on self-directed service options, and assists the person in developing a Self-Direction budget, writing Habilitation Plans and facilitating Circle of Support meetings.Fiscal Intermediary (FI):A not-for-profit agency that serves as "employer of record" for Self-Direction plans with Budget Authority; In that role, the FI conducts background checks, collects service-related documentation, pays for or reimburses budget-approved goods and services, and provides payroll/benefit services.
