There are currently nine separate apartment sites throughout Nassau County. Each apartment is centrally located allowing for easy access to public transportation, shopping, and laundry facilities. Each apartment is occupied with 2-3 adults of the same gender. They are fully furnished, along with appliances and heat/air conditioning. This treatment program is meant to foster and promote independence, along with daily support from the professionally trained staff. If needed, residents do have 24 hour access to staff.

Provides a 49-bed supervised living facility which includes personal care aides, housekeeping, case management, recreation, medication management, meals and nutrition education. Serves frail elderly who can no longer live on their own.

Offers a variety of 24-hour supervised home settings for children with severe emotional and behavioral difficulties and adults with developmental/intellectual disabilities. These programs include homes providing specialized services for individuals with Prader-Willi syndrome and Autism. Each program offers a home-like setting fostering independence and providing the opportunity for personal fulfillment for individuals.

Offers full service senior apartments. Services include: housekeeping, laundry, meals, personal hygiene help, and medication management.

Residents of the Apartment Treatment Program generally require less staff support than in the Supported Housing Program. Staff visits vary depending on the needs of residents. Usually, counselors visit residents 3- 5 times per week. However, visits can be increased should residents need additional support. All residents have 24-hour telephone access to staff in the event of an emergency. The Apartment Treatment Program provides residents with the opportunity to live independently, yet still have the benefit of frequent staff support. Residents work on areas needed for independent living such as, symptom and medication management, accessing community resources, and budgeting. The Apartment Treatment locations are situated throughout Suffolk County, Long Island, NY. All provide easy access to public transportation, stores, recreational facilities, and services. All apartments are private homes, which provide individual bedrooms to residents.

Offers residential services for people with learning and developmental disabilities to be able to receive supervision and support in community-based apartments. The program is designed to offer people patterns of daily living which enhance their growth to the maximum level of independence. Serves adults ages 18+ who reside in Nassau and Suffolk Counties who have learning and/or developmental disabilities. Call to schedule an intake appointment.

Provides individualized services to meet the needs of individuals to be supported. Services included homes with 24-hour live-in support with medical support, supported apartments with daily or less frequent visits, homes with independent apartments with only the level of support desired, living with a family in the community, and homes for children, men, women, and seniors. Offers a residential day program to support individuals by enhancing physical, social, emotional, spiritual, intellectual and occupational needs.

Provides housing units for homeless adults with disabilities and a history of high Medicaid usage as well as low income individuals and families. Services include medication management, crisis intervention, case management, assistance with medical concerns, community integration, life skills training, employment assistance, parenting skills training, financial management, education assistance, and advocacy. The Apartment Program offers a less intensive level of care for persons recovering from mental illness as participants reside in either 1 or 2 bedroom apartments located within the City of Plattsburgh. All residents have their own bedrooms, however, those living in 2 bedroom units share common space such as kitchen, living room and bathroom. Case Managers meet with residents throughout the week to offer rehabilitative support services.

Provides secure, recovery oriented living environments for individuals with psychiatric disabilities. Committed to assisting clientele to live meaningful, productive lives by providing flexible supports in a person-centered atmosphere. Residents of all programs learn the skills necessary to live independently. Treatment apartments are certified by the New York State Office of Mental Health and provide consumers with the highest level of independence. Staff make routine visits and are available 24 hours a day in case of an emergency.

Hamaspik apartment program provides supervision and monitoring from staff and community resources. Supported or community work opportunities are available for consideration in the individual’s personal planning.
