All applications for adult housing that are licensed or funded by the New York State Office of Mental Health are processed through a single location using a universal application form. Decisions about client access to housing are monitored by the state Office of Mental Health and the Nassau County Department of Mental Health with input from the agencies that provide the residential services. Administers the Housing Choice Voucher Rental Assistance Program in Suffolk County for the New York State Homes and Community Renewal and other public housing agencies. Manages the distribution of annual rent subsidies on behalf of Suffolk County residents. Rental Assistance Program assists eligible families to rent housing from the private rental housing market. Wait list is currently closed. Also administers the Nursing Home Transition and Diversion (NHTD) program and Veterans Affairs Supportive Housing (VASH) program in Suffolk and Nassau County.

Provides a variety of supports for people with intellectual and developmental disabilities, using a Person-Centered and Trauma-Informed Care approach. There are numerous homes throughout Erie, Chautauqua, Niagara, and Monroe Counties. Each has qualified staff available 24 hours a day. There are a variety of unique homes for people looking for particular services, including homes for children, specialized homes for people with complex medical needs, several apartment options, and so much more. All residential services are certified through the New York State Office for People with Developmental Disabilities.

Offers a variety of housing options, including Individual Residential Alternatives (IRAs), group-style IRAs with 24-hour on-site staff support, apartment-style IRAs in either supported or supervised settings, emergency and transitional housing for those who are homeless or in danger of becoming homeless, independent apartments, and affordable housing.

Assistance with coordination of care for seniors who live in their own homes, as well as their families. Assistance with linking appropriate resources and determining eligibility for SSI, Medicaid, Lifeline, Food Stamps, and telephone discounts. Assistance with paperwork for entitlement programs such as Medicare, Medicaid, Food Stamps, Insurance counseling etc. Education and Counseling relating to Medicare and Medicare related products. Transportation to medical appointments for an hourly fee plus mileage. Assistance with selecting alternative living arrangements, with the goals of the older adult in mind.

For in person help, please visit the satellite location at Genesee County Career Center, 587 East Main Street, Batavia. **** Provides weekly programs at the Batavia Senior Center. Some of the programs offered are bridge, line dancing, cribbage, arthritis chair-based exercise, quilters, and painting classes. New groups are always being organized. Special programs such as 55 Alive Driver Refresher Course, educational, health, and informational programs are also offered. Day trips to concerts, exhibits, musical productions, restaurants, and areas of special interest are also scheduled in the spring, summer, and fall. Senior groups and clubs exist in most areas of the county which offer luncheons and pot luck dinners. Most groups offer programs of interest, entertainment, day trips, and other recreational activities.

Provides short term, long term and transitional housing throughout Long Island in 58 group homes, emergency shelters, and single family residences for men and women. Every resident has access to intensive case management, transportation, and daily living expenses.

The program provides assistance to persons who live in a nursing home or are unstably housed with choosing, accessing, and maintaining an independent apartment.

Assists those individuals who have been chronically homeless due to disability with a permanent supportive housing program. Clients live in their own apartment and receive supportive case management services that enable them to live productive lives while maintaining permanent housing.
