Provides emergency food, clothing, and transitional housing to homeless women and children in Nassau County. Provides support counseling, advocacy and networking with multiple agencies including assistance with entry/re-entry into the job market. Additional services include case management and crisis intervention. Seeks to educate and involve the community in a concerted effort to prevent homelessness.

A transitional shelter for pregnant women and women with children who are homeless. Services include case management, advocacy, job training referrals, prenatal care, drug and alcohol rehabilitation, parenting skills, housing and economic assistance, and more.

Offers a wide variety of housing to targeted populations including homeless individuals with a history of substance abuse, and MICA, Housing for Mentally Ill Chemical Abusers. Rents are subsidized and support services in daily living skills training, relapse prevention, and entitlements intervention.

Drop-in center for homeless vets. Provides linkage with VA services and assistance with finding housing. Works with homeless and at-risk veterans to help them overcome the barriers that may prevent them from receiving or benefiting from health services. Veterans have access to a shower in the Drop-in Center. Also offers outreach to homeless veterans in shelters, soup kitchens, and on the streets. Outreach workers help establish eligibility for VA medical service and pension, develop appropriate treatment plans, and screen for community placement. Homeless veterans eligible for VA Healthcare may call for availability and screening for VA funded emergency shelter, transitional housing, and HUD section 8 voucher program. Communal supportive housing program at two secured and structured locations that provides a safe and stable environment for up to 14 adult women (18+ years old) and their children, including boys up to age 12, who are homeless or at risk of becoming homeless. Each family has a private bedroom, while common areas such as the living room, kitchen, and bathrooms are shared—fostering supportive relationships that help restore trust and encourage personal growth. This is not emergency housing, it is a longer-term structured program designed for individuals who are ready to work toward stability and independence.

Transitional residence for HIV positive persons with an AIDS specific diagnosis. Program is designed to facilitate advancement to permanent and independent housing within a limit of one year's time.

Provides temporary housing in traditional apartments located throughout the Hudson Valley. Residents from Westchester and Suffolk County can receive assistance. These settings provide families with stability and the opportunity to become self-sufficient with assistance provided by a case manager.

Provides emergency housing for families and single men in Nassau County, soup kitchens, and long term housing solutions.

Provides emergency housing through the Supervised Family Residence (SFR). The SFR houses up to nine families each, living under the supervision of a case manager. Families are able to give each other peer support and encouragement, and everyone benefits from the homelike atmosphere. CHI also provides emergency housing to single adults in a Supervised Singles Residence (SSR), licensed by New York State, using the congregate model and providing intensive case management, including supervision from a psychiatric nurse practitioner. In addition, CHI in partnership with Suffolk County DSS, operates a service center in Patchogue open to homeless single adults. Clients at the Resource Center receive help with applying for benefits, addressing mental and physical health issues, education and finding employment.

Transitional Housing Program for homeless women and children with 19 apartments. On-site support services include: culinary training access, parenting skills, advocacy, domestic violence counseling, case management, etc.

Provides shelter for homeless women and children as well as relief for individuals in domestic violence situations. Offers security 24 hours a day, 7 days a week. Stays are generally 30 days with an extension if needed. Arrangements are made for residents to move to transitional living situations in an effort to attain self-sufficiency. Provides transitional housing for women. Services provided to residents include: on-site medical care; classroom for preschool children; certified public school teachers and classroom for school age children; licensed daycare; referrals for case management, food, clothing, and shelter; links with social workers and mental health counselors; and a 12 step recovery program for drug and alcohol abuse. HOURS: Monday-Friday, 8am-4pm; Shelter operates 24 hours a day, 7 days a week.
