Transitional residential services located in a more independent community-based apartment setting with case management, vocational and clinical supports and services.

Provides a group home environment with 24-hour staff supervision with short term, focused skill development in a home-like setting. Services include crisis intervention, rehabilitation counseling, social and living skills development (money management, personal hygiene, food preparation, meal planning and recreation activities), medication management, and community integration.

Provides housing services to mentally ill individuals who are experiencing difficulties with their housing situation, and are receiving treatment in any Mental Health program. Provides a supervized apartment living program for homeless youth ages 16 to 21 years old. Serves pregnant and parenting teens and their dependent children. Offers case management to assist youth to access medical and mental health services, education and employment services.

Provides housing assistance and case management services to individuals and families diagnosed with mental illness, substance abuse and other qualifying criteria.

Provides a housing program for women and children who are victims of domestic violence and another site for single adult victims of human trafficking, and/or victims of domestic violence who were formerly incarcerated.

Provides emergency food, clothing, and transitional housing to homeless women and children in Nassau County. Provides support counseling, advocacy and networking with multiple agencies including assistance with entry/re-entry into the job market. Additional services include case management and crisis intervention. Seeks to educate and involve the community in a concerted effort to prevent homelessness.

Provides a transitional safe living environment for homeless youth, giving them the opportunity to pursue goals, learn life skills that are necessary for self sufficiency. Participants pay nominal monthly rent, and must be employed and in school, or taking part in an internship or volunteer placement.

Casa Salva provides temporary housing to homeless survivors of victims of domestic violence and their families fleeing from abusive relationships. Assists individuals to secure housing and provide a rent subsidy for a time-limited basis, with the goal of achieving housing stability and self-sufficiency.

Provides emergency housing through the Supervised Family Residence (SFR). The SFR houses up to nine families each, living under the supervision of a case manager. Families are able to give each other peer support and encouragement, and everyone benefits from the homelike atmosphere. CHI also provides emergency housing to single adults in a Supervised Singles Residence (SSR), licensed by New York State, using the congregate model and providing intensive case management, including supervision from a psychiatric nurse practitioner. In addition, CHI in partnership with Suffolk County DSS, operates a service center in Patchogue open to homeless single adults. Clients at the Resource Center receive help with applying for benefits, addressing mental and physical health issues, education and finding employment.

Drop-in center for homeless vets. Provides linkage with VA services and assistance with finding housing. Works with homeless and at-risk veterans to help them overcome the barriers that may prevent them from receiving or benefiting from health services. Veterans have access to a shower in the Drop-in Center. Also offers outreach to homeless veterans in shelters, soup kitchens, and on the streets. Outreach workers help establish eligibility for VA medical service and pension, develop appropriate treatment plans, and screen for community placement. Homeless veterans eligible for VA Healthcare may call for availability and screening for VA funded emergency shelter, transitional housing, and HUD section 8 voucher program.
