Services include on-site counseling for personal and family issues with the primary objective of enhancing the quality of work opportunities by providing educational, vocational and employment services through the Altamont Program's Vocational, Educational and Employment Services, job placement and job retention services. Veterans can make the facility their home for a period of 6 months.

Services include landlord mediation, utility company mediation, referrals, access to safe and affordable transitional housing. Households are counseled on restructuring debt, establishing reinstatement plans, foreclosure prevention strategies, loan forbearance, management of household finances, obtaining rental subsidies, landlord and renter rights, the eviction process and locating alternative housing. Financial assistance also consists of security deposit assistance, rent payment assistance, and assistance with back utilities and back rent. Transitional housing is located in Almond and is only for individuals or families who are facing homelessness.

Develops and manages affordable rental housing for farm workers and also provides technical assistance to growers, municipalities and other non-profits to facilitate the development and improvement of affordable housing for migrant and seasonal farm workers.

Provides a temporary bed to individuals being released or who have been recently released from the county jail that have a diagnosed mental illness and are homeless.

Angels for Warriors connects veterans with the resources they have earned from their service to our country. We assist Veteran's transition to permanent housing, register them for veteran benefits and advocate on the veteran's behalf for medical care.

Provides temporary housing for adults transitioning out of homelessness, and assigned a case manager to assist with obtaining community services and pursuing the goals of achieving self sufficiency and permanent housing.

Provides transitional housing for homeless families with a broad range of social services to help them achieve financial independence. Services include case management, housing placement assistance, day care, food pantry, youth leadership training, medical/physical health assessment and an advocacy program. Also provides referrals for other services that may be needed.

This program is a 90-180 day transitional housing program for VA eligible veterans. Supportive case management services and linkage to VA medical and support services are provided as well.

Provides a transitional safe living environment for homeless youth, giving them the opportunity to pursue goals, learn life skills that are necessary for self sufficiency. Participants pay nominal monthly rent, and must be employed and in school, or taking part in an internship or volunteer placement.

Provides emergency housing through the Supervised Family Residence (SFR). The SFR houses up to nine families each, living under the supervision of a case manager. Families are able to give each other peer support and encouragement, and everyone benefits from the homelike atmosphere. CHI also provides emergency housing to single adults in a Supervised Singles Residence (SSR), licensed by New York State, using the congregate model and providing intensive case management, including supervision from a psychiatric nurse practitioner. In addition, CHI in partnership with Suffolk County DSS, operates a service center in Patchogue open to homeless single adults. Clients at the Resource Center receive help with applying for benefits, addressing mental and physical health issues, education and finding employment.
