This waiver program assesses, plans, and supports the transition of nursing home and long term care facility residents, who are eager to and capable of returning to community living, back to integrated community living settings of their own choosing. JUVENILE UNIT | ROCKLAND COUNTY DEPARTMENT OF PROBATION

Juvenile unit provides diversion services, in collaboration with Social Services, Mental Health, Volunteer Counseling Services, Mental Health Association and other agencies to Persons in Need of Supervision PINS and Juvenile Delinquents JDs and monitors juvenile cases referred to Family Court. Also, provides supervision to youths placed on probation by the Family Court and liaison services to the non-secure shelter.

Provides case management, makes referrals to other agencies and offers mediation to Nassau County youth ages 10-17, and their families who are referred through the Nassau County Juvenile Probation Department.

ALTERNATIVES FOR YOUTH | EAC - HAUPPAUGE

Provides a PINS diversion program. AFY is a 30-45 day home based crisis intervention for youth that display a pattern of incorrigible behavior(Defiance in the home, disobeying curfew, aggressive behaviors, truancy/failing school and mental health issues). Case Managers will do an assessment of family needs and links them with community resources to get the family out of crisis. In addition, after the 30-45 days are completed. a Phase 2 of the program is implemented to link the families with partnership agencies for typically 3-6 months, but can be up to a year to ensure the ensure the family's needs are met and they remain stabilized. AFY also provides parent support workshops, adolescent male and female workshops as well as peer mentoring services.

A 6-month program where convicted individuals have an opportunity to get help without going to jail or prison. This program must be completed in order for the individual to be set free of jail/prison time by doing a variety of services, such as community service, schooling, job training, alcohol/drug treatment, if applicable and intense counseling.

Offers a temporary home for 11 to 18 year old children who are court mandated. Provides a school program, individual counseling, and group dynamics. The Our Lady of Peace Academy is a free standing alternative school for grades 7 through 12. Individualized instruction and private tutoring is provided by qualified teachers.

Services include, but are not limited to, anger management, school advocacy, life skills coaching, communication skills, job assistance, self -esteem, self-awareness and parent education. Program is for parents and children together.
