Offers care coordination and Cognitive Behavioral Intervention services to individuals being released from state prison or a local county jail to Erie County through the Enhanced Release Under Supervision Program. These programs assist individuals in reacclimating to the community while addressing behavioral and physical health needs, obtaining entitlements such as medical insurance, and decreasing recidivism. ##REENTRY##

The New Dawn Initiative, sustained through Peaceprints of WNY’s Project Blue program, provides reentry-focused support for high-risk incarcerated individuals with co-occurring mental health and substance use needs. Participants work weekly with a case manager to assist in the development of individualized case plans and can engage in services offered directly through Peaceprints and community partners or through linkages to community-based organizations. Available supports may include housing assistance, education and employment services, mental health and substance use treatment, public benefits assistance, and other reentry-related services. Provides support services for individuals who are incarcerated and/or transitioning from prison life back into the community.

Consists of a collection of integrated education and counseling services designed to facilitate the successful re-entry of ex-offenders into society. Re-ROUT strives to reduce the recidivism rate of criminal offenders who have completed a period of incarceration and/or probation in Suffolk County. Realizing that the first few weeks post-release can be crucial to a successful transition process, participants are encouraged to make contact with the Re-ROUT office located on the BOCES Ward Technical Center campus in Riverhead as soon as possible after release. There, the ex-offender is introduced to the Re-ROUT services. In the weeks that follow, the Re-ROUT office offers the needed services, referrals and support to aid the client's reintegration. New York Probation Officers and Correction Officers inform probationers and prisoners about the Re-ROUT Program during the initial orientation sessions. As part of the In-Jail Education Program, the Re-ROUT counselors work with the Probation Department and immediately help interested clients develop career/life/education plans. Upon release from the correctional facility and/or probation obligations, the ex-offender is invited to report to the Re-ROUT office at the Bixhorn Technical Center (BTC) in Bellport. There, the ex-offender is introduced to the Re-ROUT "outside" counseling staff. In the weeks that follow, this staff provides the needed services, referrals and support to aid their client's reintegration into society.

Offers alternatives to detention and state incarceration, supports youth post-adjudication to help with compliance and other needs, and provides reintegration support for youth transitioning out of these placements.

The WYARP is designed to provide job readiness through a 4 week course, occupational skills training, educational assistance, and employment services to young adults 18-24 that are justice involved for Mount Vernon Residents living in zip code 10550.

The hotline is staffed by inmates employed by Federal Prison Industries (UNICOR) who are specially-trained to provide assistance to recently released federal inmates as they navigate various reentry challenges and seek community resources.

Offers assistance to released released inmates with a disability that are transitioning back into the community. Provides advocacy to help former incarcerated individuals with disabilities find employment, housing, VR services, gain independence living skills, and social supports.

Offers offender re-entry programs which are available to those being released from a correctional facility, to aid in their transition back into the community.

Provides representation on civil matters related to previous incarceration such as employment discrimination, Section 8 issues, certificates of relief from civil disability, good conduct, housing issues, public benefits, and consumer debt. Referral required. Call for further information.

A non-profit agency providing free adult education and community resources in basic reading, writing, math, digital literacy/computer skills, English instruction, GED prep, ESOL and civics, workforce readiness, vocational training with certifications, and more. Classes and tutoring services are provided by trained volunteers and agency staff. Also provides jail transition services to assist with housing, public assistance, mental health and substance abuse, and other pertinent issues. ##REENTRY##

Inmate look-up, parolee look-up, and most-wanted look-up information. Help in filing complaints against mistreatment of individuals who are incarcerated. Re-entry program which assist in a smooth transition in returning home from prison with housing, mental health and medical treatment and services, and more. Assist individuals to find out about parole processing and appeals.

Offers offender re-entry programs which are available to those being released from a correctional facility, to aid in their transition back into the community.

Provides case management for non-violent ex-offenders ages 16-55. Assists clients with clothing, transportation and other personal needs. Provides support services to clients with the goal of making them more productive community members. Offers information and referral to appropriate agencies to address client needs. Assists clients with job readiness and placement in appropriate training programs affiliated with local unions, BOCES and adult education. Seeks job placements for participants and provides tokens for transportation.

A program which helps state inmates transition from the corrections system back into their community, helping them re-connect with needed services and support.

Works to identify justice-involved Veterans and contact them through outreach in order to facilitate access to VA services at the earliest possible point. Provides a support program for criminal offender suffering from serious mental illness to successfully transition and reintegrate into the community upon their release.
