Assists high school students with disabilities, assess their objectives for adult community life. The program encourages students to think about their future and develop skills to communicate their ideas with parents and teachers, and help develop more independence, self-esteem, and skills needed for a productive and fulfilling future. A team effort is implemented with families, schools, government services, and the general community to help students reach transition goals. Information, training, and technical assistance is available for students, to help with planning, benefits and entitlements, assistive technology, reasonable accommodation, personal assistance services, community resources, and housing options.

PRE-VOCATIONAL SUPPORT | NEW YORK SCHOOL FOR THE DEAF

Provides pre-vocational services, beginning in elementary school, with career education being infused across the curriculum in various activities. Students in the upper grades receive a range of services, such as transition counseling, vocational assessment which is geared toward helping students acquire the skills necessary to succeed in society and in their community, and are delivered both in the classroom and in the community.

Individual Family Supports - Intandem

Provides behavior management, counseling, crisis intervention, family reimbursement, guardianship assistance, benefits and entitlements assistance, home modification, independent living skills, information and referral, parenting skills for parents of developmentally disabled children (in or out of home), service coordination/case management, support groups, and transitional planning for persons with developmental disabilities of all ages. Developmentally disabled parent training is provided to first time parents with a developmental disability. Applied Behavioral Analysis (ABA) Program available for parents of children with autism.

Special Needs Achievement Program (SNAP) - St. Mary's School for the Deaf

To meet the needs of students with multiple disabilities, SNAP provides students a small class milieu in a Total Communication, Language Rich Environment (LRE). Instruction, adapted from the New York State Education Department standards, is provided using Total Communication - spoken, signed and written language - to meet students' individual communication needs. Student progress is assessed, pending eligibility, by the New York State Alternative Assessment (NYSAA). The goals of the program are to prepare students to care for themselves; to develop social awareness and social skills which will enable the individual to function as social beings in the community; to achieve emotional growth which will enable the individual to maintain the lifestyle of choice; to develop motor skills; to develop personal and home management skills required for self sufficiency; to select and prepare for an occupation; to develop intellectual skills which will lead to responsible and independent citizenship; and to develop the ability to use leisure time well and in a way that will enhance creativity and enjoyment of life. Related services, including Speech, Physical and Occupational Therapies, as well as counseling are available. Audiological services such as hearing tests and minor device repair (hearing aids, cochlear implants, etc.) are provided on-site by licensed audiologists. Adapted physical education is also available. A therapeutic after school program is available, as is a 5 day residential program. Emphasis is placed on Daily Living Skills and transition from school to adult life.

TRANSITION SERVICES | SOUTHERN ADIRONDACK INDEPENDENT LIVING CENTER

SAIL provides information, advice, and assistance to youth with disabilities transitioning from school, family members, and other interested parties concerning transition planning and adult services and supports. The center also provides advocacy for transition planning, and assistance with the CSE process. SAIL provides assistance and support to individuals who are facing transitions in their lives. This includes transitions from Early Intervention Programs to school-based programs, transitions from school to work and/or adult services, transitions from living at home to moving into individual living arrangements, and other life transitions.

REACH PROGRAM | ST. CHRISTOPHER'S - DOBBS FERRY CAMPUS-EXECUTIVE OFFICES

A multi-faceted program which teaches appropriate behaviors and help students who are cognitively limited with intensive treatment needs, including those with autism spectrum disorders to develop the cognitive, social and life skills required to interact in the larger world. Also prepares students for an educational and employment plan, through individualized academic classes, job support and internships. Provides the transition support when students are in the process of going from education to employment.

CRISIS ORIENTED RESPONSE FOR DEVELOPMENTAL DISABILITIES (CORDO) | WESTCHESTER JEWISH COMMUNITY SERVICES (WJCS)

Offers individual, family and group counseling, behavior management as well as information and referral for individuals with intellectual and developmental disabilities and autism and their families. A therapeutic, adult, social, and recreational club meet weekly to work on goals that include increasing socialization and self-advocacy skills. Provides coordinated transitional services and training to help students move from school to post-school life in order to live as independently and self-directing as possible.
