Provides a comprehensive, multidisciplinary outpatient program for children and adolescents with asthma. The team includes sub-specialists in Pediatric Pulmonary Medicine and Allergy/Immunology. Treatment includes pulmonary function testing, complete allergy testing and treatment. At the Asthma Learning Center patients and families can learn about asthma through reading, watching videos or playing interactive games

CHILDREN AND FAMILY TREATMENT AND SUPPORT SERVICES - CFTSS | WELLLIFE - CORAM

These therapeutic, rehabilitative peer support services are open to children and youth, ages 5 through 21, who are covered by Medicaid and have mental health and/or substance use needs. These services give children and their families the power to improve their health, wellness, and quality of life. They can be provided in the home or community. Services offered include: Licensed Practitioners for in home therapy, Community Psychiatric Supports and Treatment, Psychosocial Rehabilitation, Family Peer Support and Youth Peer Advocacy and Training.

Youth Link Program - OCH

Provides primary and specialty care, care coordination, individual and group counseling, health education, self-management training and treatment adherence for adolescents ages 13-24 years of age. Team specializes in care for youth and young adults who are living with HIV or at risk for HIV, sexually transmitted infections (STIs), who are experiencing homelessness, mental health and/or substance use. Also provides comprehensive and supportive service for youth who identify LGBTQ.

Robert Warner, MD Center for Children With Special Needs - OCH

Provides comprehensive rehabilitation services consisting of diagnostic, evaluative, and therapeutic services to children with known or suspected disabilities. Center also supports the Regional Perinatal Center of WNY, and the only Level III Neonatal Intensive Care Unit (NICU) in Western New York that has the full scope of pediatric subspecialists required to operate a Level III NICU. Each center or program is comprised of an interdisciplinary team of specialists specifically selected to meet patients' needs. Services are family focused and provided in conjunction with pediatricians and community resources. Clinics offered include: Motor Clinic, Spina Bifida Clinic, Spasticity Clinic, Early Neurodevelopmental Clinic, Comprehensive Developmental Clinic, Feeding Disorders Clinic, Down Syndrome Program, Speech and Hearing Program, Orthopedic Rehabilitation Program, Physical Therapy, Occupational Therapy, Adaptive Seating Program and Aquatic Therapy.

The Childrenâs Rehabilitation Center is an outpatient pediatric diagnostic and treatment center that provides comprehensive medical and rehabilitative services to children ages birth to 21-years-old with complex physical and developmental disabilities throughout Westchester, Putnam, Rockland, Dutchess and Orange Counties and the Bronx. The Childrenâs Rehabilitation Center is located on the same site as the White Plains campus of the John A. Coleman School.Services include: physical, occupational, speech and language pathology, aquatic and feeding therapies. Medical specialties include, but are not limited to: orthopedics, physiatry, ophthalmology, neurology, neonatology, pediatrics and audiology. In addition, brace, orthotic and adaptive equipment clinics, psychological and social work services, and Care at Home case management services are provided.

HEMOPHILIA TREATMENT CENTER | COHEN CHILDREN'S MEDICAL CENTER NORTHWELL HEALTH

Provides comprehensive multidisplinary services to children and their families with hemophilia, Von Willebrand disease and related bleeding disorders. Staffing includes two hematologists (one pediatric, one adult), and orthopedist, two nurse clinicians, genetic counselor, social worker, physical therapist and a dental hygienist.

Provides a number of programs and services for medically fragile and technology dependent. This facility containsts the largest freestanding pediatric ventilator program in the country, along with skilled nursing, rehabilitative care, and a state-of-the-art respiratory care program. In addition, provides an on-site private school and day care for medically fragile and technology dependent children.

HOME AND COMMUNITY BASED SERVICES - HCBS | WELLLIFE - CORAM

HCBS assists youth with a serious social, emotional and/or behavioral challenge to remain at home by offering an array of community based services to prevent placement in inpatient levels of care or Residential Treatment Facilities. Services provided would enable a child already in such a placement to be discharged more quickly.
