This includes information about the services and supports available in their local community. There are transition specialists at Independent Living Centers in counties all over New York State. Suffolk Independent Living Organization houses the Open Doors Transition Center and Peer Program for all of Long Island. Transition specialists will meet with you and/or your family members to identify what services you will need and help you in getting those services.Open Door's Peer Program is a group of people who have lived in a nursing home and intermediate care facility and are now receiving services and support locally available to them. Peers visit nursing homes and developmental centers to share their experiences with residents and support people who want to move to the community.

  • Diabetes Prevention Program: For those with prediabetes or at high risk for type 2 diabetes. A year long program focused on implementing lifestyle changes to reduce the risk of developing type 2 diabetes.
  • Diabetes Counseling: For individuals with prediabetes or diabetes. Provides one-on-one visits with a diabetes care and education specialist to discuss individual needs and support.
  • Diabetes Self-Management Classes: For any individual with diabetes. Classes focus on the fundamentals of diabetes self-management, covering topics such as understanding diabetes, blood sugar monitoring, nutrition and exercise, acute and chronic complications, and diabetes medications.
  • Diabetes Support Group: For any individual with diabetes and those supporting them. Meets once every other month to share struggles, celebrate successes, and discuss topics related to diabetes management.
