- Diabetes Counseling: For individuals with prediabetes or diabetes. Provides one-on-one visits with a diabetes care and education specialist to discuss individual needs and support.
- Diabetes Self-Management Classes: For any individual with diabetes. Classes focus on the fundamentals of diabetes self-management, covering topics such as understanding diabetes, blood sugar monitoring, nutrition and exercise, acute and chronic complications, and diabetes medications.
- Diabetes Support Group: For any individual with diabetes and those supporting them. Meets once every other month to share struggles, celebrate successes, and discuss topics related to diabetes management.
Provides assistance with basic needs for individuals or families who have emergency need. Services provided, as resources permit, include emergency rent payment assistance, clothing, utility assistance, emergency food vouchers, rent and security deposit on a limited basis (excluding water/sewer bills), and personal hygiene items (shampoo, toothpaste, sanitary items, diapers), which can be utilized once every 30 days while supplies last. Offers food vouchers from the Utica office, and a food pantry for individuals and families in the Rome area. Assists with prescription drug assistance for up to a month supply, depending on fund availability to Madison County residents.
