A comprehensive health center offering primary care (assessment, treatment, and referral), dental clinic, health education, and diabetes education. Tuscarora Community Health Worker Program provides home-based case management services to participating women and their children and helps them overcome barriers to care. Workers help identify complicating factors that make it difficult for clients to access the care they need; assist in enrolling pregnant women into prenatal care and children into primary care; assist in enrolling for services such as WIC, Medicaid/PCAP and Child Health Plus; provide education on topics such as breastfeeding, baby care, lead poisoning, immunization, nutrition, HIV/AIDS, domestic violence, and substance abuse; provide referrals to food pantries, housing assistance, educational programs, childcare, and other services; and provide support and help to overcome the barriers that prevent families from receiving a healthy start in life. Federally Qualified Health Center offering OB/GYN, social work, and nutritional counseling. Serves all populations with special focuses on the medically under-insured, uninsured, and those experiencing homelessness. Also provides free walk-in pregnancy testing, onsite lab, ultrasound, STD testing, health education/groups, blood pressure screening, transportation, and referral assistance. Provides transportation assistance to help clients get to appointments and referral assistance. ##REENTRY##

Provides programs exploring interests ranging from computer training, arts and crafts to nutrition and fitness. Popular courses include aqua aerobics, using the Internet, and investment planning. Provides environmental information, products, and a home safety survey by Health Inspectors/Educators. Home visits include an assessment for indoor air quality, asthma control, carbon monoxide poisoning, fire and burn safety, lead poisoning hazards, unintentional injury, smoking cessation, and housing and property maintenance. Following the home assessment, intervention products and services are provided according to need and interest. Free products include: smoke detectors and batteries, CO detectors, child safety products, cleaning supplies, asthma trigger management supplies, and radon test kits to qualifying families.

The objective is to provide a forum through which participants can share their experiences and discuss matters of common interest relating to prostate problems. Survivors assist newly diagnosed patients with information on treatment choices. Meetings are held at St. Joseph's Hospital in Cheektowaga on the third Tuesday of each month. Provides a caring community that understands the challenges of living with diabetes. Helps individuals to obtain knowledge, skills, and support to better manage prediabetes and diabetes. Programs include: - Diabetes Prevention Program: For those with prediabetes or at high risk for type 2 diabetes. A year long program focused on implementing lifestyle changes to reduce the risk of developing type 2 diabetes. - Diabetes Counseling: For individuals with prediabetes or diabetes. Provides one-on-one visits with a diabetes care and education specialist to discuss individual needs and support. - Diabetes Self-Management Classes: For any individual with diabetes. Classes focus on the fundamentals of diabetes self-management, covering topics such as understanding diabetes, blood sugar monitoring, nutrition and exercise, acute and chronic complications, and diabetes medications. - Diabetes Support Group: For any individual with diabetes and those supporting them. Meets once every other month to share struggles, celebrate successes, and discuss topics related to diabetes management.

Coordinates efforts of VA professionals from various clinics and departments including eye clinic, low vision rehab, primary care, veterans benefits and others organized by the VIST coordinator to serve veterans with legal blindness, severe visual impairment and access disability. Coordinator provides case management services, prosthetic evaluations and training, adjustment to vision loss counseling, and linkage to resources and professional counseling. ##Veteran##
