Provides home safety visit to identify hazards or areas of concern, provide environmental information, create a home safety plan with recommendations for improvement, and provide products to carry out hazard reduction. Home visits include an assessment for hazards related to fire safety, lead poisoning prevention, indoor air quality, carbon monoxide poisoning, radon, ventilation and odors, cleaning and clutter, pests, mold/mildew and moisture, asthma, and injury prevention. The Healthy Neighborhoods Program (HNP) is designed to help improve the personal health, safety, and living conditions of individuals and families to reduce the burden of housing related illness and injury. Program provides home safety visits include an assessment for hazards related to fire safety, lead poisoning prevention, indoor air quality, carbon monoxide poisoning, radon, ventilation and odors, cleaning and clutter, pests, mold/mildew and moisture, asthma, and injury prevention. Following the home assessment, staff work with the resident to create a plan to make their home safer. Staff provides information, recommendations, and products to correct or reduce hazards. Products may include: smoke detectors, carbon monoxide detectors, replacement batteries, radon test kits, fire extinguishers, asthma management tools, green cleaning items including baking soda and vinegar, child safety products such as door knob covers, outlet covers, and cabinet latches.

A comprehensive health center offering primary care (assessment, treatment, and referral), dental clinic, health education, and diabetes education. Tuscarora Community Health Worker Program provides home-based case management services to participating women and their children and helps them overcome barriers to care. Workers help identify complicating factors that make it difficult for clients to access the care they need; assist in enrolling pregnant women into prenatal care and children into primary care; assist in enrolling for services such as WIC, Medicaid/PCAP and Child Health Plus; provide education on topics such as breastfeeding, baby care, lead poisoning, immunization, nutrition, HIV/AIDS, domestic violence, and substance abuse; provide referrals to food pantries, housing assistance, educational programs, childcare, and other services; and provide support and help to overcome the barriers that prevent families from receiving a healthy start in life. The University at Buffalo has created a questionnaire to help researchers understand experiences of people who have been infected with COVID-19. The University at Buffalo is trying to learn of the many experiences and health problems that occur after COVID-19 infection in Western New York. The questionnaire gives patients an opportunity to share their story, symptoms and experience with COVID-19 and long COVID.
