Navigators are now available to speak with individuals and families to provide education about the different types of health insurance plans offered through the New York State of Health (NYSoH) Marketplace and to assist with enrollment into public insurance programs and qualified health plans.

Provides information, counseling, and assistance with the most up-to-date information about Medicare, Medicare Supplemental Plans, Managed Care Plans (HMO, PPO, PEES, MSA), Premium Rates, Medicaid, Prescription Coverage, and Long-Term Care Insurance. Helps people make educated decisions regarding insurance options by providing unbiased, up-to-date, personalized information. Helps uninsured/under-insured persons. Also provides confidential help with health insurance problems, including billing, filing appeals, referrals to appropriate agencies including legal assistance, and one-on-one counseling by appointment. Group presentation available. Provides training for professionals and caregivers.

Administers Medicaid program to assist eligible individuals in our community in obtaining necessary and appropriate medical care through programs such as Medicaid, Medicaid Social Security Income (SSI), Child Health Plus, Personal Care Supports including nursing and personal care aids, managed care insurance, and long-term care supports, among others. Child/Teen Health Program is a preventive health program which provides continuing medical care to your children. Also provides for burial when a Temporary Assistance recipient or other indigent person dies leaving no funds or insurance sufficient to pay the cost and there are no relatives, friends or other persons liable or willing to take responsibility for the burial expense. Office serves as a Cooling Center during times of excessive heat. Always call ahead to make sure a Cooling Center is open before going there.

Lists are printed of drug providers, contact information and neighborhood pharmacies. Social Security Extra Help applications will also be available.

The NY Medicaid Choice Program provides Managed Health Care education, enrollment and disenrollment for all Medicaid managed health care beneficiaries. The NYS Managed Care Enrollment Broker (MAXIMUS) administrates the program for Rockland County residents.

SRH provides applications and assistance for state insurances for children and families. Also assists with other questions regarding a variety of insurance issues.

Provides information, counseling and education on long term care insurance and on the NYS partnership for long term care. Helps individuals assess whether it is appropriate for them, and if so, provides information on how to choose a policy.

Senior Benefits Information Centers (SBICs) help adults 60 and older, and those caring for the elderly, achieve a better understanding of Medicare health plans and prescription drug coverage (Part D prescription drug coverage, Medicare Advantage, Medic-Gap). Information and counseling is provided about different types of government programs, including federal and state financial assistance programs, and county-wide benefits. You can meet one-on-one with a trained counselor at one of our library-based locations. Spanish interpreter can be made available by appointment.

Offers insurance enrollment programs and seminars for individuals or families needing medical insurance. These services are available at Nassau University Medical Center and at our Family Health Centers, with convenient day and evening hours. We have organizations available to help you enroll. Citizens Inc. provides Medicaid Service Coordination(MSC) to people with developmental disabilities. Works with the person in the creation of an Individualized Service Plan (ISP) which outlines in detail every support to be accessed by and for the person. The plan is reviewed on a regular basis and updates are made as needed. They maintain steady communication with all service providers, provide links to services and supports, advocate for the person, and document all coordination activities.

Medicaid Application Assistance Program (MAAP) offers Medicaid application assistance. Facilitated Enroller can assist with the process of applying for Medicaid including going over the application, help to compile all the information needed to apply, submit the application and necessary documents to the local Department of Social Services.
