INDEPENDENT CONSUMER ADVOCACY NETWORK (ICAN)/MEDICAID FACILITATED ENROLLMENT | WESTCHESTER DISABLED ON THE MOVE

In collaboration with New York State Community Service Society, assist people who are enrolled in New York State Medicaid manage care plans and those who receive long-term care services; such as questions regarding Medicare and Medicaid programs for those receiving long-term care, managed care option, help to resolve problems with plan and providers, help in lodging complaints or appeal health care decisions. Medicaided Facilitated Enrollment Program is also offered for the aged, blind and disabled to assist in enrolling and receiving insurance programs, such as Medicaid, long term care (for homecare) enrollment, Medicaid recertification, Medicare Savings Program.

MEDICAID HOME CARE | RIVERHEAD CENTER

Individuals with both Medicare and Medicaid, who are 21 years of age or older, seeking information on various Medicaid home care programs should call New York Medicaid Choice at 1 (888) 401-MLTC (6582). Other individuals not enrolled in Medicaid Managed Care seeking information regarding non-institutional care such as personal care aide services, should call the Home Care Intake Unit at (631) 854-9585. The Independent Consumer Advocacy Network (ICAN) is an independent, conflict-free entity providing Individuals free assistance in accessing care, filing grievances, appealing adverse decisions, and understanding and exercising rights and responsibilities. There are 60 physical locations throughout NYS for in-person assistance, a call center (1-844-614-8800) for the entire state, and a website icannys.org. ICAN currently serves MLTC plan enrollees and mainstream Medicaid managed care enrollees who receive long term care. Call for detailed information.

FACILITATED ENROLLMENT FOR THE AGED, BLIND AND DISABLED - AOP | ACTION FOR OLDER PERSONS

A public health insurance application assistance program for people who are aged, blind or disabled. We educate and assist New Yorkers with enrolling in and using public health insurance programs. Facilitated Enrollment consists of private, one-on-one counseling about Medicaid and the Medicare Savings Program. We can meet with people at our offices, various public locations, by phone or at a client's home if they are unable to travel. Our service areas include Chenango, Cortland, Delaware and Otsego counties.

MEDICAID MANAGED CARE PROGRAM | SMITHTOWN CENTER

All Temporary Assistance, Medicaid-only and SSI-Medicaid clients, may choose from five managed care plans to receive the complete menu of Medicaid Benefits. There are no costs, no caps, and no co-payments. Each member has his/her own physician or pediatrician-OB/GYN, who will coordinate all their health needs.
