In partnership with health care providers, Community Health Workers provide intake, referral, navigation and case management services to patients at Primary Care Practices.

raises funds to support sight and hearing projects, including guide dogs sponsors Empire State Special Needs Experience (formerly Empire State Speech and Hearing Clinic) in Spencer offers financial assistance to those needing eye exams and/or glasses or hearing aids takes donations of used eyeglasses and hearing aids through drop boxes in optician's offices, Lifelong and some churches free vision screening for children one year and older at day care centers, Head Start, schools, and other programs. Done with Plus Optix Screener, a sophisticated scientific, clinical instrument.

The Seniors Program connects those 65+ with local volunteer ophthalmologists who provide a medical eye exam often at no out-of-pocket cost, and up to one year of followup care for any condition diagnosed during the initial exam, for the physician services.Medicare/other insurance are billed and accepted as payment in full, resulting in no out-of-pocket expense to the patient. Uninsured individuals are not charged.Does not cover eyeglasses or medicine.EyeCare America is a public service program of the American Academy of Ophthalmology. Its goal is to provide uninsured and underinsured individuals 18 years and older with access to medical eye exams. It does this by matching patients with a volunteer ophthalmologist in their area. Ophthalmologists are the only eye doctors with a medical degree.
