Facilitates a smooth transition from the hospital to home for Nassau County children under 18 years of age with mental illness. Provides crisis assessment, individualized planning and family needs assessments. Offers family support by a treatment team to ensure adequate discharge planning and access to community-based services to promote long term symptom management and recovery.

Provides services to the Jewish community including Holocaust survivor assistance services, counseling and support, supports for Hebrew Day Schools, and Project 5.
