EverCare at Home professionals consult closely to the patient, caregiver and the physician to create an individualized care plan for home care services that meets individual needs and addresses their desires. These services include physical, occupational, speech therapy, nutritional evaluation and planning, skilled nursing, home health aides, social work, medical supplies and equipment and appliances.
