Provides supports to people with developmental disabilities to develop or enhance the skills needed to live more independently in their homes or their community. Being a part of a community is an essential part of living a full life. Community habilitation gives people who have disabilities the skills they need to become a more active part of their community, build relationships with others and to simply, have fun.

NYS OMH licensed residential program that provides 24 hour supervised congregate setting for 8 adolescent boys with a primary mental health diagnosis. Provides case management, support in daily living and the development of independent living skills, counseling, recreation, medication management and understanding symptoms, family support and relationship building.

Offers assistance to individuals with development disabilities by helping them find housing that fits their personal needs. Services include peer role modeling, opportunities for self-help, personal empowerment, and growth.

Offers supervised and supportive housing in the community for people with chronic mental illness and for those with developmental disabilities.

Offers community habilitation, respite, and intensive behavior services for individuals with Autism Spectrum disorders or a related developmental disability.

Helps homeless and transient youth and young adults, many of whom have dropped out of school, move from dependence to self-sufficiency.Participants can receive: outreach services assistance locating safe housing intensive case-management independent living skills training crisis management parenting skills training paid apprenticeships to help them develop skills necessary to secure unsubsidized work Housing Scholarship program, transitional living program in the Youth Outreach Program, houses homeless and transient youth and young adults houses in scattered site apartments. Rent and utilities paid for 12 to 18 months while services and support are offered to: develop skills necessary to maintain stable housing identify and pursue educational and career pathway to provide a livable wage become productive members of the community

* Promotes independence, as the Fiscal Intermediary, for individuals with developmental disabilities by allowing individuals to create and manage their own budgets, hire their own support staff, and engage in activities that will enhance their ability to live and work in the community.

* Provides individuals with developmental disabilities choices to control their care including:

• Employer authority that enables individuals to hire, manage, and dismiss workers;
• Budget authority that provides the individual with a flexible budget to purchase the supports and services needed to live independently;
• Daily activities such as paid or volunteer work, continuing education, and recreation;
• Support: family, coworkers, friends, support staff;
• Where to live
• Set the rhythms of daily life: what time to eat, when to go to sleep, etc.

Provides life-skills and assists individuals with obtaining important documents such as driver's license, social security card, and birth certificate.

Offers community habilitation, respite, and intensive behavior services for individuals with Autism Spectrum disorders or a related developmental disability.

Provides temporary care to children who are unable to remain with their family. Caseworkers provide the services and support necessary to reunite the family. If a child cannot safely to return to their family, the agency works to develop a permanent plan for the child.

Advocates can answer questions and provide assistance with applications for Social Security Disability and SSI, Temporary Assistance, HEAP, Medicaid, Food Stamps, and housing. Advocates can also assist recipients of DSS with understanding sanctions and the steps for Fair Hearings.
