A six-week workshop that helps participants learn or improve problem-solving and decision-making skills, enabling them to confront the challenges of living with diabetes. Participants learn to improve food choices, increase physical activity in an easy and effective way, and master coping skills to control their diabetes. Caregivers are also welcome.

To provide activities and resource centers for children (grades K-5) in the Hudson Valley who are coping with a serious family illness or crisis Pediatric Chronic Illness Support Group- The Pediatrics Staff provides group activities designed to enhance the well-being of chronically ill patients and their parents.Autism Support Group- Is a support group for parents affected by Autism Spectrum Disorder. This support group is designed to meet the psychosocial needs of the parents. ADHD Support Group - Is a support group for parents of children with ADHD, Juvenile Diabetes Support Group, Type 1 Diabetes or Juvenile Diabetes.

Plans and implements wellness health screens and educational programs for health professionals and the community. These programs and services are offered in response to community need. All health screen participants receive follow-up and assistance with referral, if needed. Screenings and programs include a tobacco-free program, blood pressure screenings, free diabetes screenings, and disease management programs.

This program aims to help participants improve knowledge, skills, and confidence related to their condition. This program is not a substitute for diabetes education with a healthcare professional team, but is a supplemental program that can help individuals and their family members successfully manage their health and maintain an active and fulfilling life.

DSMP workshops are free to attend, and are designed for people who have Type 2 Diabetes and receive Medicaid, and meet once a week for 2.5 hours, for six weeks.LHVPN and the WMC Health PPS have partnered with six organizations in the Hudson Valley to offer Diabetes Self-Management Program (DSMP) workshops. These organizations include CLUSTER, Inc. (Yonkers), Yonkers Community Action Program (YCAP, Yonkers), Open Door Family Medical Centers (Westchester County), Konbit Neg Lakay (Spring Valley), Meals on Wheels Programs and Services of Rockland (Nyack), and Rehabilitation Support Services, Inc. (Dutchess, Orange, Sullivan).
