• Provides home care services and transitional services to individuals with a variety of needs. The level of care provided depends on the need, which is outlined below: Home Health Aide (HHA) - Provides assistance with feeding, dressing and bathing. HHA's monitor temperature, pulse rate, respiration rate, and blood pressure. Personal Care Aide (PCA) - Provides health care tasks, activities of daily living including personal hygiene services, housekeeping tasks essential to the patient's health and other related supportive services. Companion/Homemaker - A program that provides assistance with house cleaning, laundry, meal preparation, reading, conversation, grocery shopping, prescription pick-ups and errands. Private Pay Care - Private Pay Care encompasses any of the above services. Contact office for private pay rates if not contracted with your insurance company. Skilled Nursing - Provides skilled nursing services such as medication management, diabetic management, wound changes, catheter care, injections. Vary depending on agency availability.

• Provides assistance in maintaining an independent lifestyle for individuals with disabilities with the following resources:
  • Self-Direction - for persons with developmental disabilities
  • Medicaid Service Coordination
  • Habilitation Services - assistance with daily tasks and independent living skills
  • Consumer Directed Personal Assistant Program
  • Assistance in transitioning from nursing care facilities to the home (including moving assistance).
  • Respite vouchers to allow caregivers to attend to their personal needs.
  • Financial reimbursement for items that allow an individual with disabilities to remain in their home.

• Provides services to assist those who are chronically ill or have a physical disability and want to live at home, but need skilled nursing services and/or help performing everyday tasks.
• Allows the flexibility and freedom for individuals to choose, hire, train, and supervise the staff/caregivers they invite into their homes.

• Helps individuals of all ages and their caregivers find services and supports to help meet the goal of remaining in the community as independent as possible for as long as possible.* Provides callers with screening, benefits eligibility assistance, information and referral, and long-term care options counseling regarding available services and supports.* Provides assistance navigating the long-term care system and facilitate linkages to services such as home care, transportation and meals.* Provides free complete in-home assessment or evaluation of individuals needs, care plan development, community PRI assessment (screening for nursing home placement), and access to Medicaid homecare and Office for Aging programs such as caregiver support, respite and home care (EISEP).
