Services include open and supportive discussions about treatment choices and symptom management; coordination of care with all health care providers; emotional support for the patient and family; 24/7 telephone access to a registered nurse; social work visits to help complete advance directives and connect with community resources; spiritual care support; routine visits and regular phone contact to monitor the patient's condition and answer questions by the palliative care case manager.
Focuses on the relief of symptoms and stress associated with serious illness, whatever the diagnosis, with the goal of improving quality of life for patients and their families. Palliative Care is appropriate at any point in an illness and can be provided along with other treatments.
