Offers supported employment opportunities which include job development, job coaching, person centered vocational profiles, vocational assessments, vocational planning, counseling, employment, job training, job placement, and transitional support to school age students.

Offers short term career training programs focusing on employment skills and placement services for visually impaired individuals. Both online and in-house courses are available which focus on computer skills, customer service skills, telecommunications, and work readiness. Emphasis is placed on computer skills including Microsoft Office, Assistive Technology, and hands on experiential learning. Also provides employment services including resume development, interviewing skills training, career counseling, and job search/placement support. Program is open Monday-Friday, 8:30am-4pm in-house; specific course hours will vary, or be available as online offerings.

Creates job opportunities in the community for people with qualifying disabilities. Services include intakes, assessments, follow-ups, job readiness training, vocational assessments, job development, job placement, and job coaching.

Offers a number of programs designed to meet the needs of children with developmental disabilities and their caregivers. Services include:

• Respite Services: Offers planned short-term and time-limited opportunities for families and care providers to have some personal time.
• Employment Services: Helps adults and youth to obtain and retain competitive employment through vocational counseling, employment training programs, work experience acquisition, soft skills development, rehabilitation services, and support services to help youth obtain, maintain, or advance in competitive employment or self-employment.
• Children's Health Homes/Care Management: Includes care coordination, comprehensive transitional care, individual and family support, and referral to community and social support services as needed.
• Community Habilitation: Assists youth living at home with a loved one or independently in the community to maintain or develop the highest level of independence possible for them.
• Community Based Day Habilitation: Promotes community inclusion and integration.
• Family Reimbursement Program: Covers expenses for goods and services needed to care for a loved one with a developmental disability who lives at home.
• Self-Directed Services: Enables individuals to make choices about their staff, supports, and services.
• Preventive Services: Provides support to help families learn the necessary skills to ensure their children remain at home and out of foster care placement.
• Behavioral Health Services: Includes community habilitation, caregiver/family supports and services, pre-vocational services, and supported employment.
• Project C.O.A.C.H.: Provides telephone counseling, referral, outreach, and intervention/prevention for individuals and families.

Provides a variety of services in home and community based settings to assist with improving the quality of life for people with disabilities and their families via OPWDD and OMH. Supports include respite, community habilitation, recreation, after school programming, summer programming, broker services, early intervention services, children's mental health services, intensive parent training, children's health homes, and employment services.

Supported Employment: Services include pre-employment, placement, post-employment, case management, crisis intervention, and job coaching. Transportation may be available depending upon placement; not always available. Vocational Assessment Services: Services include vocational evaluations, career counseling, pre-employment testing, and linkage. Driver's Rehabilitation: Services include training, clinical evaluations, and vehicle modification evaluations, available for individuals with severe limitations.

The Employment Connection, a division of UCP of Long Island , assists individuals with developmental disabilities and those with acquired physical disabilities to meet their career goals. Assists job seekers with employment assessment, training, job placement and job support.

Offers developmentally disabled individuals within Jefferson County an opportunity to work in the community in paid and volunteer positions. Helps individuals identify their strengths, skills, and interests and develops opportunities to match the person with the right job, as well as employment supports.

Program offers services, assistance, information, advocacy and referral for those needing help navigating special education services, community habilitation, pre-vocational and employment programs, independent living skills and peer-to-peer support.

Works to empower individuals with developmental disabilities, youth and adults with chronic health conditions and complex mental health concerns, and survivors of domestic violence to live as independently as possible. Adult Services include:

• Day Services, including site-based day habilitation, community-based day habilitation, and Individual Retirement Experience (IRE) for those aged 50 and over

Provides employment training opportunities for adults with disabilities. Services include situational assessment, supported work, job coaching, job placement assistance, and facility based employment.
