This signed statement must provide an additional explanation, or causal pathway, linking the cause of death listed on the death certificate to COVID-19. If the death certificate occurred on or after May 17, 2020, the death certificate must attribute the death directly or indirectly to COVID-19.

Child/Teen Health Program is a preventive health program which provides continuing medical care to your children. Also provides for burial when a Temporary Assistance recipient or other indigent person dies leaving no funds or insurance sufficient to pay the cost and there are no relatives, friends or other persons liable or willing to take responsibility for the burial expense. Office serves as a Cooling Center during times of excessive heat. Always call ahead to make sure a Cooling Center is open before going there.
