Marjorie Doyle Rockwell Center and Eddy Alzheimer's Center at Hawthorne Ridge are the only medical model adult day services in the Capital Region designed specifically for older adults with memory loss who are still living in their own homes or in the community with caregivers. Adult Day Programs offer a full range of health and social services to help older adults with physical, emotional, or mental impairments, or who require supervision and/or medical care. Services enable older adults to remain independent for as long as possible, while allowing caregivers relief from the responsibilities of at-home care giving so they can fulfill work or other obligations. Transportation is provided in geographic area. Lunch and snacks are also provided for participants.

Offers many different types of services ranging from personal care assistance to nursing services, as well as physical therapy.

* Provides Veteran patients with general health care and social needs to allow them to continue living at home or with family. Services include: Therapeutic exercise. Recreational activities. Health education & medication management. Meals. Care for patients with dementia. Support and information for family caregivers.

Provides medical services to adults in a community day setting. Services include medication administration/treatments by LPN/RNs, case management, physical therapy, occupational therapy, and speech therapy, ongoing daily activities, and lunch and nutritional services overseen by a registered dietitian.

Hearing evaluations . Lab and diagnostic studies. Mental health referrals. Transportation. Socialization.* Provides recreational services: Recreation therapy: music, arts & crafts, gardening and more. Field trips and outings to the mall, museums, theater, etc. Visits from area school children. Group exercise and individualized weight training/walking programs. Religious and educational activities. Trips to the hair salon.

Provides personal care services including health and medical care, case management, health assessment, physical rehabilitation therapy, occupational therapy, speech therapy, and nutritional education as well as activity programs throughout the day.

* Provides day programs (structured, day habilitation, adult day social) for individuals with brain injury; social skill development, peer support, compensatory strategies and cognitive training. Evening recreational opportunities.* Offers vocational Services that provides job placement and training.

* Offers day habilitation services. Social and recreational activities, while receiving assistance with personal care and other areas of need. Individuals work toward goals and objectives that are derived from their valued outcomes.* Provides at-home assistance to individuals to help them become increasingly independent and able to live in the least restrictive environment possible.* Offers weekday and weekend respite. Weekend Respite provides an opportunity to socialize and enjoy a variety of recreational activities. Assistance with personal care, meal times, medication and other areas of need.* Cayuga Centers is granted funding to assist families with costs of goods or services directly related to the care of a family member with a developmental disability.

Provides assistance to help older adults ages 60 and older who are not Medicaid eligible and their families delay or avert costly and premature placement in nursing facilities. Services include case management services and non-medical supportive services which can include aide service for light housekeeping and personal care as well as respite. A consumer directly model, where the care receiver hires someone they know, is also offered.

• Adult Day Services - Provides nutritious meals, social programing, supervision, personal care services, community outings, assistance with activities of daily living and door to door transportation (Oneida and parts of Herkimer counties only).
• In-Home Companion Care - Offers companionship and assistance with everyday activities.
• Caregiver Services - Provides community education, information, and referral for caregivers of the disabled or elderly.
• Ombudsman Services - Provides information and advice to Nursing Facility residents who have issues with their services.
• Open Doors Transition Program - Informs Nursing Home residents of community based living options and explains transition process.

Round-trip transportation is available and participants may attend up to five days a week. Eddy DayBreak programs include medical, social and dementia-specific models.
