Eddy DayBreak is an innovative adult day service that helps improve quality of life for adults who still live independently, or are being cared for by family members. Offering supervision and/or assistance with personal and/or medical care, Eddy DayBreak programs provide caregivers relief from at-home caregiving, allowing them to fulfill work or other obligations. Round-trip transportation is available and participants may attend up to five days a week. Eddy DayBreak programs include medical, social and dementia-specific models.
* Offers a day program for adults with physical, psychosocial, and medical needs. Encourages self-esteem, social interactions, and independence to keep the participants with their caregivers in the community. Social work. Activity programs. Assistance with daily medication. Personal care and hygiene. Nursing (on-site lab work, wound care, acute medical needs). Arrangement and transportation to and from medical appointments. Physical, speech, and occupational therapies. A hot noon meal that can accommodate special diets. Foot care.* Transportation is provided to and from the program within Monroe County.

Provides assistance to help older adults ages 60 and older who are not Medicaid eligible and their families delay or avert costly and premature placement in nursing facilities. Services include case management services and non-medical supportive services which can include aide service for light housekeeping and personal care as well as respite. A consumer directly model, where the care receiver hires someone they know, is also offered.

The Wilkinson Adult Day Health Center is a community-based long term care program that provides comprehensive health care services in a congregate day setting. Clients are provided with the medical and social services they need to remain home and part of the community. St. Mary's Adult Day Care is offered at The Wilkinson Residential Healthcare Facility on the Memorial campus. Adult Day Health Care is designed to meet the health care needs of chronically ill, frail elderly, and disabled adults who require certain primary, preventive, diagnostic, therapeutic, rehabilitative or palliative services.
Provides medical services to adults in a community day setting. Services include medication administration/treatments by LPN/RNs, case management, physical therapy, occupational therapy, and speech therapy, ongoing daily activities, and lunch and nutritional services overseen by a registered dietitian.
Provides care to people who need long term care services and help to live independently in their own homes and communities.A wide range of services are available to suit individuals and their circumstances. Services include nursing, home health aides, therapy, medical supplies, adult day social care, non-emergency transportation, nutrition care with home delivered meals and much more.
