Care Management services for Adults and Children who have physical and/or mental health diagnosis. Care Managers will assist with scheduling appointments, arranging for transportation and will attend appointments at the clients request to help advocate for the clients wants and needs. A Care Manager for a child will work with the parents or guardian to ensure a child is safe, healthy and well cared for.
