* Provides Home Health Care and Non-Medical Home Care Services including:

• RN Home Health Care Assessment
• Home Health Aide Services

• Dementia/Alzheimer's Care
• Companionship: meal preparation, light housekeeping, laundry and transportation

Volunteers may provide transportation to essential services and appointments, run errands, assist with grocery and personal shopping, or pick up prescriptions, plan a regular phone visit or home visit to provide friendly and supportive listening, prepare/deliver a weekly meal or treat, perform general yard work or light housekeeping, and/or provide respite for caregiver.

* Provides transportation (or completes the errand for the person) for essential errands like grocery shopping and medical appointments as well as other supports for people to stay independent in their own homes.

* Provides licensed professional/paraprofessional assistance to older adults who require care in their homes. Services include: Customized care plans. Post surgery monitoring. In-home respite. * Assists seniors and individuals with disabilities with supportive in home assistance not typically funded by traditional supportive agencies and services.

Provides a comprehensive program of in-home services for functionally impaired individuals 60 years of age and older. Services include case management, in-home services (housekeeping and personal care), in-home respite, and ancillary services.

Home delivered meals are available to seniors unable to get to the dining centers. Transportation is also provided to the dining centers, shopping assistance, information and referral services, and recreational activities. Performs skilled personal care and homemaking services for those recuperating from acute illness or a disability. Each case is overseen by a registered nurse and services are tailored to meet each client's individualized needs.

Provides skilled nurses and home health aides to meet a variety of medical and personal home health needs. Care is available in the home, nursing home, or hospital from one hour up to 24 hours per day depending on need, and availability.

Brings together people of many faiths to help their neighbors in need. A national program made up of volunteers that shop, drive or just check in on individuals with long-term health needs in order to help them stay in their homes longer.

Provides assistance to help older adults ages 60 and older who are not Medicaid eligible and their families delay or avert costly and premature placement in nursing facilities. Services include case management services and non-medical supportive services which can include aide service for light housekeeping and personal care as well as respite. A consumer directly model, where the care receiver hires someone they know, is also offered.

* Provides an informal support to individuals via a network of volunteer service exchanges services include:

• Basic computer help.
• Cleaning (bathroom, kitchen, vacuum, garage, fridge, stove)
• Replacing smoke alarm or CO2 detector batteries.
• Weeding and or garden help.
• Taking Screens off and or putting storm windows on.
• Hang pictures, help with decorating.
• Minor household repairs.
• Replacing light bulbs.
• Laundry.
• Turning mattresses.
• Raking/trimming.

Provides personal care, housekeeping/chore services, and case management for the frail elderly.

An alternative to "traditional home care/personal care", CDPA allows eligible individuals to hire and direct their own personal care workers (including most family members, a neighbor or friend), to assist with bathing, dressing, eating, and other daily tasks. Eligibility is determined by the appropriate county entity. CDPA believes there's no place like home.

Provides assistance to help older adults remain in their homes. Case managers coordinate services, which can include aide service for light housekeeping and personal care, a personal emergency response system, and respite.

College and high school students offer assistance to senior residents (65+) in the towns of Owego and Apalachin on a designated Saturday in the spring. Contact Sr. Mary for sign-up. Volunteers rake leaves, complete other outdoor yardwork, wash windows, inside and out (first floor only), do interior cleaning, vacuuming, etc.

Senior helpers are paid by the people requesting the jobs. People interested in hiring are encouraged to screen senior helpers.

* Offers someone who can assist with a listening ear or a helping hand when the primary caregiver cannot be there.* Provides respite care that offers short term breaks that relieve stress, restore energy, and promote balance in your life for the family.* Provides shopping assistance for groceries, personal items, gifts and prescriptions.* Provides meal preparation, laundry service, making beds and changing linens, tidying up, running errands, and medication reminders.* Offers transportation to doctors' visits, dialysis procedure, trips to the hairdresser or barber, or just visiting a friend.

Offers home health services that range depending on level of care needed. Service options include meal planning/preparation, medication reminders, transportation/errands, housekeeping, companionship, grooming/hygiene, and bathing assistance.

* Provides individuals the connection to assistance necessary to stay in the home including:

• Non medical personal care
• Housekeeper/chore services
• Case management
• Information and referral services

*Allows individuals the opportunity to choose what services they receive through Consumer Directed EISEP.

*Provides linkage to outside agencies who will perform these services.

Provides care for seniors and the physically challenged offering assistance with daily living, medication management, protective oversight, light housekeeping, socialization and transportation to appointments.

Home-aid provides assistance with light housekeeping, errands and shopping for home-bound elderly who are unable to perform these tasks themselves. Assessment and case management are provided. Self-referrals accepted.
