Offers In-home services and supports funded under the Expanded In-home Services for the Elderly Program (EISEP) and administered by Nassau County Office for the Aging and its subcontractor agencies. Care coordination is a person-centered planning process that takes place between a qualified case manager/care coordinator, the applicant, and, often, members of the applicant’s informal support network. Care coordination begins with a comprehensive assessment that identifies an individual’s needs, abilities, preferences and available resources. Next, a care plan is developed based on the findings of the assessment. The care plan is a coordinated package of services and supports derived from multiple sources to best meet the applicant’s needs. An alternative to "traditional home care/personal care", CDPA allows eligible individuals to hire and direct their own personal care workers (including most family members, a neighbor or friend), to assist with bathing, dressing, eating, and other daily tasks. Eligibility is determined by the appropriate county entity. CDPA believes there's no place like home.
