Provides adult respite care for developmentally disabled individuals, age 21 and over, who reside in Suffolk/Nassau counties with their families. Limited geographic area may be eligible for transportation periodically. Universal medical application and site visit appointment required.

DIAGNOSTIC AND EMERGENCY GROUP HOME | HOPE FOR YOUTH

Provides 12 co-ed beds to youth in need of either a diagnostic evaluation, emergency placement, or a crisis/preventive respite stay. The youth are supervised 24 hours a day in a professionally staffed setting and are engaged in an academic program during the school year. Evaluations provided include psychological, psychiatric, psychosocial, and substance abuse risk assessments as needed.

FAMILY CAREGIVER SUPPORT SERVICES | SUFFOLK COUNTY OFFICE FOR THE AGING

Services including case management, information and referral, counseling, support groups, training and respite have been developed under Title III-E of the Older Americans Act. This program helps sustain family caregivers in their efforts to care for an older relative who has a chronic illness or disability. The Office for the Aging coordinates a variety of services. These services are designed to complement, not replace, family care. Priority is given to caregivers 60 years and older with the greatest social and economic need. Hearts & Hands - Faith In Action, Inc. (Service Group)

Hearts and Hands is a volunteer based non-profit organization that provides free escorted transportation, companionship, and in-home supports for older adults in communities throughout Erie and Eastern Niagara Counties. Our care recipients are often individuals who stopped driving due to age-related medical conditions or for financial reasons. Recipients may also need assistance at home with light yard work and home maintenance, or companionship to renew and refresh their caregiver. Our elderly neighbors are often lonely, isolated, and in need of friendly human interaction. In response to a recent community assessment and a national assessment from AARP, Hearts and Hands has designed our program to address social isolation through all three avenues of service: transportation, household supports, and companionship. Hearts and Hands volunteers may be the only people our care receivers see during a week, and their company is, in and of itself, a bright spot in daily life. Hearts and Hands is a best-efforts program. Services are provided by volunteers who give their time as they have it available. WILLING HEARTS, HELPFUL HANDS | PARKER JEWISH INSTITUTE FOR HEALTH CARE AND REHABILITATION

An innovative community program serving family caregivers by connecting them with a circle of help that includes trained volunteers who provide an array of respite services. Our well-trained volunteers provide family caregivers of older adults with the time to meet self-care needs, as well as other family obligations, at no cost. RISING TREETOPS AT OAKHURST | RISING TREETOPS AT OAKHURST

Provides periodic respite breaks for primary caregivers while disabled relative is cared for at Rising Treetops at Oakhurst. Disabled family member lives among peers and and has access to seasonal recreational activities including crafts, athletics, swimming, games and TV. Also offers overnight respite where registered families can reserve time in advance. Day respite is available to residents of Monmouth County.

* Provides respite care for families who have children with emotional, behavioral, and/or mental disorders. Planned group respite events takes place in the community. These events are generally offered to identified children and siblings, on a regular schedule, predetermined by the parents and staff. In-Home and Community respite care is available on a planned hourly basis to all of the children in the home or at a community activity.
